It still has bristles. But it's not gripping the way it did when it was new. Studies that dismantled brand-new toothbrushes found 30–120 microplastic particles already embedded in a brand-new toothbrush head, before any use particles per head before anyone had brushed with them, and projected annual exposure at roughly 48,910 projected microplastic particles per person per year from toothbrush use alone particles per person from toothbrushes alone Protyusha et al. 2024. Simulated-brushing studies then measured ongoing release — up to 39 microplastic particles released per day during simulated brushing (Öztürk Aytulun et al. 2025) particles a day in one seven-day trial Öztürk Aytulun et al. 2025, and into the thousands per cycle in another Fang 2023. The literature is young and hasn't yet published a clean "new versus worn" shedding curve, but the mechanism points one way: fatigue that splays a bristle is fatigue that fragments it. Replacing on time is a microplastic decision as much as a plaque one — which is a strange thing to have to say about a toothbrush.
The subscription question: whose clock is the three months on?
The three-month interval has become a business model. Many bristle brands — manual and electric alike — ship a replacement head or brush on a recurring subscription timed to that window, which is genuinely convenient and, on cleaning grounds, sound: a fresh bristle really does clean better. It's worth seeing the whole picture, though. The clinical clock and the commercial clock happen to line up at three months; that alignment is convenient for both you and the seller.

Boar and horsehair bristles run on the same two-to-three-month replacement window, but they're keratin: they shed no microplastic while you use them and biodegrade as protein at the end.
