Asked plainly — how long do boar bristle toothbrushes last? — the answer is two to three months. That is what is on our own product page, and it sits inside the range any dentist would give you for any brush. The more useful question is what the number is actually measuring, because it is not the point at which the brush stops cleaning your teeth.
Nearly every clinical trial that has put a worn toothbrush up against a new one has struggled to find the cliff people assume is there. That does not make the interval arbitrary. It moves the reason for replacing a brush away from cleaning performance and towards the condition of the head and what is growing on it. So this is about what wear looks like on hair rather than plastic, why the handle darkens every week, what genuinely shortens a natural-bristle brush's life, and what happens to it afterwards. Does a three-month-old toothbrush clean worse than a new one?
Tan and Daly ran the tidiest version of this experiment in 2002. Forty adults attended the clinic twice, each visit following 48 hours of no oral hygiene at all, and brushed for sixty seconds — once with a new brush, once with the brush they had been using for three months. Both reduced plaque by roughly 34%. The old brushes were graded as showing minor, moderate or marked wear, and the grade made no significant difference. The authors concluded that three-month-old toothbrushes were as effective as new ones.
A larger trial pushed the age far past three months. Van Palenstein Helderman and colleagues gave 101 children aged seven and eight two brushing sessions, one with a new brush and one with their own 14-month-old one. The new brushes did leave less plaque — 10.9% less, statistically solid. But the researchers had set their non-inferiority threshold at 15% before starting, and 10.9% sat inside it. After more than a year of use, the old brushes were still working within the margin the authors had defined in advance as acceptable.
Sforza and colleagues followed twenty students for three months, half swapping to a fresh brush monthly and half keeping the same one throughout. Plaque scores rose in both groups with no significant difference between them. The more interesting line is the gingival index: it rose significantly in the group that never replaced their brush, and fell just short of significance in the group that did. The between-group comparison was still not significant, so it is a hint rather than a finding — but the hint points at gums, not plaque.
The study that pulls the other way is Conforti and colleagues in 2003, and it is worth knowing what was in the test: Oral-B's own manual and battery brushes, after three months of twice-daily use. New and worn brushes both reduced plaque significantly. The advantage for new brushes showed up at approximal sites — the surfaces between the teeth — and among the most extremely worn heads. The conclusion was that brushes should be replaced before the bristles reach that state.
Read together, the honest position is narrower than either camp would like. On the flat outer and inner surfaces of your teeth, a three-month-old brush cleans about as well as a new one. Between the teeth, and at the extreme end of wear, it probably does not. The American Dental Association's line is "toothbrushes should be replaced approximately every three to four months or more often if the bristles become matted or frayed," which is a sensible convention rather than a measured threshold — the reference the ADA attaches to that sentence on its own page is a 2013 Cochrane review of school-based behavioural interventions for preventing tooth decay, which is not a study of bristle wear. What does wear look like on natural hair?
Nylon is an extruded polymer. It is drawn to a constant diameter, cut to length, and it holds whatever shape you bend it into. Wear on nylon is a permanent set: the outer filaments lean outward and stay there, and the cut ends flatten and curl. That is why an old plastic brush looks so obviously old. The starburst is the polymer remembering every stroke you ever made.
Boar bristle is keratin — the same protein as hair — and it behaves like hair. It takes on water and softens, then stiffens again as it dries. It does not hold a set as readily as nylon, so a worn boar brush usually never develops that dramatic splay. What it does instead is lose spring. The tuft stops pushing back against your thumb. That signal is subtler than a starburst and it is the one worth learning, because a boar brush can be past its useful life while still looking perfectly tidy in the holder.

There is a claim repeated on almost every article about this: splayed bristles damage your gums. We went looking for the evidence behind it and it is not what people assume. The strongest trial on bristle geometry and gum abrasion is Hennequin-Hoenderdos and colleagues in 2017, which compared brushes with 0%, 40–50% and more than 90% end-rounded filaments under controlled professional brushing. The non-end-rounded brushes produced significantly more abrasions at the gum margin. But that is about how filaments are finished in the factory, not about what three months in your mouth does to them. As far as we can find, nobody has tested wear-induced splay against gum abrasion directly. The inference is reasonable. It is still an inference.
Why does the bamboo handle go darker?
This is the thing people email us about most. The handle is uncoated moso bamboo — no polyurethane lacquer, no paraffin wax, no sealant of any kind — and uncoated bamboo darkens when it is repeatedly wetted and dried. What you are watching is tannin oxidation: compounds already present in the wood reacting with air and water, going amber and then brown, usually fastest where your fingers grip it. It is the same reaction that turns a wooden spoon dark over a year of washing up.
It is not mould, it is not rot, and it is not the brush telling you its time is up. A lacquered handle stays pale for longer, which is exactly why most bamboo brushes are lacquered — keeping the wood looking new is a cosmetic decision that also puts a petroleum-derived film against your lips twice a day. We would rather the handle went brown. Judge the brush by the bristle field, not by the colour of the wood.
What actually shortens a natural-bristle brush's life?
Moisture with nowhere to go. Natural hair holds water in a way nylon does not, so the single largest variable in how a boar brush ages is whether the head dries between uses. Stood upright in open air it is dry within a couple of hours. Lying flat in the puddle at the edge of the sink, or sealed into a plastic travel case, it is still damp when you next pick it up.
The evidence on sealed storage is better than you would expect for something so mundane. Mehta, Sequeira and Bhat found 70% of used toothbrushes heavily contaminated, and that fitting a plastic cap made things worse rather than better — specifically increasing growth of opportunistic organisms including Pseudomonas aeruginosa. Frazelle and Munro's review of the literature lands in the same place: brushes stored in closed containers yield higher bacterial counts than brushes left open to the air, and bacteria survive beyond 24 hours wherever moisture is present. The ADA compresses all of it into one instruction — "store toothbrushes in an upright position after use and allow them to air dry" — and adds that a closed container promotes microbial growth more than open air does. If you travel with a brush, the case needs ventilation slots or it is doing the opposite of its job.
What shortens a natural-bristle brush's life
- Sealed plastic travel cases — the head goes in wet and stays wet; a vented case or nothing at all is better
- Storing it flat, or head-down in a cup that collects water in the bottom
- A closed, unventilated bathroom cabinet — warmth without airflow is the worst combination
- Rinsing under hot water rather than warm; keratin is a protein and heat does it no favours
- Brushing hard, which flattens a soft tuft far faster than time does
- A shared holder where heads touch, which transfers contamination between brushes
Pressure deserves its own line. Boar keratin is soft, so leaning on it is self-defeating — the tuft folds instead of cleaning and you get through brushes faster for nothing. The Bass technique dentists have recommended for seventy years, a 45° angle and short gentle strokes, is also the brushing style that lets a natural head reach its full interval.
When should you replace it?
Two to three months is the calendar answer, and if you would rather not think about it, use the calendar. The brush itself gives better signals than the date does. Press the bristle field against your thumb: if the tuft folds over and does not push back, the spring has gone and that is the real end point. Look at the outer tufts from the side — leaning is normal, splaying flat is not — and at the tips of individual hairs, which split the way hair does. Replace it early, without deliberating, after any illness that involved your throat. None of those signals is the colour of the handle.
What happens to it at the end?
Both halves compost, on quite different clocks. Pull the tufts out of the head first — pliers, or just wiggle them free; they are held in by compression rather than glue, so they come out as clumps rather than shedding loose. The bamboo handle breaks down in garden soil in about six months. The keratin bristles take twelve to eighteen. Nothing from either part needs to go into general waste, which is unusual for something you replace four or five times a year.
That is the part of the answer with nothing to do with brushing. A plastic brush on the same schedule is four or five pieces of polypropylene and nylon a year, per person, and the nylon sheds microplastics throughout its service life rather than only after you throw it out. On a plastic brush the replacement interval is a disposal problem. On this one it is a gardening job. Do boar bristles wear out faster than nylon?
Not faster — differently. Nylon takes a permanent set, so it splays outward and looks worn while still feeling stiff. Boar keratin softens and loses its spring without changing shape much, so it can be finished while still looking tidy. The practical consequence is that you cannot judge a natural brush by eye alone the way you can judge a plastic one. Press the tuft against your thumb; if it folds and stays folded, replace it.
What happens if I keep using one for six months?
Based on the trials, less than most people expect on the cleaning side — worn brushes have repeatedly removed plaque about as well as new ones on the flat tooth surfaces, and one study found old brushes non-inferior even at 14 months. What accumulates instead is contamination on a head that never fully dries, and a tuft with no spring left that does progressively less between the teeth. That is why the interval exists, and it is a hygiene argument rather than a plaque one.
Does a darkening handle mean the brush is finished?
No. Darkening is tannin oxidation in uncoated bamboo — the wood reacting with water and air — and it starts within the first fortnight, long before the bristles have done anything at all. A handle can be deep brown on a brush that has six good weeks left in it, and pale on one whose tuft is already dead. The two are unrelated: the colour tracks how often the handle has been wet, and the bristle field tracks how much you have brushed.
The short version: replace it every two to three months, judge it by whether the tuft still pushes back rather than by the date or the colour of the wood, dry it upright in open air, and compost both halves at the end. The interval is a hygiene habit, not a cleaning cliff — the literature is reasonably clear that a worn brush still removes plaque from the surfaces it can reach. Where it loses ground is between the teeth, and no toothbrush of any age was ever good at that. That is what floss and interdental brushes are for.
